Behavioral Health

is designed to help individuals with psychiatric illness manage their care. To accomplish this goal, clients are provided with individually tailored plans of care, which focus on reducing symptoms and helping them function more independently within the community. By coordinating services with the client, family and health care providers, clients are assisted in managing their care within the least restrictive environment-the home.

Services provided by the team may include:

  • Skilled Psychiatric Nursing

  • Physical, Occupational and Speech Therapy
  • Nutrition Services
  • Respiratory Therapy
  • Home Health Aide Services
  • Durable Medical Equipment

The program is designed to:
  • Reduce hospital length of stay
  • Reduce the frequency of re-hospitalization
  • Increase adherence to treatment regimes
  • Provide education about illness, health maintenance, medications, coping skills, diet, safety issues, use of community resources
  • Assist individuals with psychiatric illness to reach maximal health, safety and independence within the community setting
